Personal Health Kit
Your Personal Health Kit is designed to help you inform your care team and manage your condition. Information from your customized kit and Care Plan will be analyzed to provide the best care possible.
Easy to set up and easy to use, just be sure to plug in the Tablet and keep it plugged in. Turn the tablet on, and follow the “Quick Set Up” instructions in the box. All the personal health devices are battery operated. After set up and putting in your PIN Code of four numbers, you must remember your PIN code for the next time you are scheduled to do something. Simply touch the screen when you are reminded to do your care plan with sounds or glowing buttons.
Personal Cell Phone (+Go)
The Vivify Pathways +Go service lets you use your own smartphone to participate. Getting started is as easy as clicking the link in the invitation text or email.
Once enrolled in this program, just wait to receive your first text message and click the link. After your first welcome message, verifying your identity and accepting the program, you will receive text messages with links that will direct you to answer short questions, enter your health data, or read/watch educational information on your condition. That’s it!
Your Remote Care Team looks at your health information from your kit every day, because it is securely sent to them over the cellular network. Your team is immediately alerted if your measurements are too high or too low, or if there is an indication you are not doing well and may need some help. You may be contacted by phone or video call, when they need to talk with you.
